Transport assist/travel

We offer support for our clients with transport attending specialist appointments, community access programs, daily life activities and much more. Our lovely staff can ensure a safe ride to your desired location in a fully comprehensively insured vehicle and can also support you in learning the process of booking taxis and other alternate transports.

Community participation

Including supported shopping, medical appointments, sporting and recreational events, social activities, visiting or making new friends, travel and transport and building confidence and social skills;

Assist with Personal Activities

Such as housework, laundry, shopping, personal hygiene, dressing, preparation or supply of meals and medication assistance.

Behavioral Support

Positive behavior support including assessment the development of a plan developed that aims to limit the likelihood of behaviours of concern developing and /or increasing

Participate In Community

Our goal is to support our clients in connecting with community support networks, cultural programs, social and recreational activities and transitioning to independence to be able to utilize these supports autonomously.

Support Coordinator

Assist in providing Support within a specialist framework necessitated by specific high level risks in the participant’s situation. Support is time limited and focuses on addressing barriers and reducing complexity in the support environment, while assisting the participant to connect with supports and build capacity and resilience. It may also involve development of an intervention plan which will be put in place by disability support workers.

Vehicle modification

Vehicle modifications mean changes to a vehicle, or the installation of equipment in it, to enable a participant to access it and, in some cases, operate it.

Community nursing care

This is the provision of specialist care for participants who have high care needs.

Interpreter services

Assistance to the participant to enable independent communication in essential personal, social or community activities where translation is not available from a mainstream service.

Plan Management

This support focuses on strengthening the participant’s ability to undertake tasks associated with the management of their supports. Plan management is the financial management of the plan supports including making payments to providers, expense claims processing, developing monthly statements for participants and claiming for payment from NDIA.

Developmental Life Skills

This support aims to assist clients (and family members where applicable) to create an effective and therapeutic work environment where a client’s capacity has the ability to increase.

We mentor clients step by step by supporting them in developing skills such as:

  • Budgeting / financial management
  • Participation in group activities Vocational skills training
  • Pro-social modelling and mentoring Daily routines
  • Hygiene routine
  • Community participation integration Communication in group settings

With this support we also teach appropriate communication skills to identify, evaluate and self-develop alternative solutions to problems. We believe that learning this knowledge is crucial, as it allows clients to autonomously continue developing other further life skills without lapsing into dependency on others.

Group Centre Activities

Assisting participants to participate in group based community, social and recreational activities.

Home modification

Home modifications involve changing structures, layout or fittings to allow an older person or someone with a disability to safely move around their home. Such modifications can:

  • support independence
  • reduce the risk of injury.
